New Member Process
Referrals sources or potential members can download a Referral Form.
Once completed, please send the form to the Program Director: Stephen Chaffee
Referral Forms can be accepted by email, mail, or fax (please call ahead before faxing - our machine is public).
Once received, an Enrollment Coordinator will reach out to schedule tours, and then orientation.
Tours and Orientations are scheduled at either 9:30am or 1:30pm every Tuesday and Thursday. If you need accommodations please call Elliot House and let us know.
(There are no tours or orientations scheduled the week of holidays or major Elliot House events).
